Carelon Behavioral Health, Cigna, Evernorth Horizon Blue Cross and Blue Shield, Kaiser (Out-of-Network), Optum, Providence Health Plan, Quest Behavioral Health, Select Health- Colorado, UnitedHealthcare UHC | UBH. Private pay rates are $185 an hour for individuals and $250 an hour for couples.
